Root Cause Analysis

Root cause analysis is an essential risk reduction and elimination technique. It concentrates on removing the causes rather than symptoms of adverse events.

Most incidents have multiple approaches to resolution with research having proven that incidents are 95% related to process and only 5% related to personnel. Despite such research most organisations spend more time looking for culprits rather than causes thus opting for the most expedient solution, treating symptoms rather than underlying causes. Such an approach makes it inevitable that the incident will recur.

Analysing root causes to eliminate future errors

Root Cause Analysis is the in-depth investigation of an incident or series of incidents to find the real reason underlying their occurrence. It is only when the underlying or root cause is identified that an informed  decision can be made as to whether it would be more economical to remove that cause or simply continue to treat the symptoms.
